Our liability for treatment is governed by the provisions and limitations of                                        
the Industrial Commission of Arizona (ICA).
Employees are required to report an accident and any injuries to their immediate supervisor 
and call the Alliance Triage Hotline at 1-888-252-4689 within 24 hours of the injury.
