The Business Card Request Form pinpoints specific changes needed to generate a personalized card within our Standardized format. After submitting the form, you will receive an email confirmation.

You can order MPS Telephone Directory for yourself, school or department. 

Please fill in the MPS Telephone Directory form and submit. You will receive an emailed response with your order information for your MPS Telephone Directory. You should receive your order in about 5 working days.

Pricing:

$2.50 per directory
