Forms and Approval Process
Paperwork: Complete the school grant request form and submit it to your principal. Make sure you download the PDF before completing it.
Delivery: items will be delivered directly to your school.
Use: the items belong to Mesa Public Schools for the purpose you submitted in the grant request. If you leave your school or the district, the items do not transfer with you.
Paperwork: send electronic copies of the grant request form and associated list of items (i.e. packing list) to firstname.lastname@example.org
Delivery: items are being delivered directly to schools. Be prepared to accept and distribute the items.
